Case Manager - FSP
Case manager needed for Full Service Partnership program. Duties include ongoing intensive case managment of 10-15 mental health/co-occurring disorder patients situated in the community including referrals, follow-up, crisis intervention, on-call services, engagement, retention and outreach. Candidate must have a working knowledge of Department of Mental Health documentation requirements and be computer literate.

FT/PT Chemical Dependency Counselor INTERN-Unpaid. Train with an experienced Certified AOD Outpatient Supervisor; will sign for hours. Provides the unique opportunity to work closely with and learn from a multidisciplinary team.
